Register for Online Services

An Online Services Account will enable you to:

  • Book appointments at your GP Practice
  • Request Repeat Prescriptions

Please note that you must be aged 13 years and over to register for Online Services.

There are two ways you can register for Online Services as follows:

Method 1 – NHS App

Use the NHS App online to access services on your PC or laptop computer, through your web browser. With NHS App you can verify your identity online if you have a webcam camera on your PC or laptop. You can also access NHS App online on your smartphone or tablet.

Method 2 – Apply to your GP Practice

Apply to your GP Practice using the Register for Online Services Form below. Before starting your application, please note the following:

  • In order to register for online services the practice requests that your identity to be verified with the following:
    • Photo ID
    • Proof of address dated within 3 months
  • The online registration process allows you to upload files of your ID documents as attachments to the Register for Online Services Form and these documents will transmit to the Medical Practice with the form.
  • Before completing the Register for Online Services Form, have your files containing your ID documents (either scanned documents or photo images) ready to upload and attach to the form
  • If you are unable or do not wish to upload files with your ID documents you should still complete the online Register for Online Services Form. If you choose this option, the practice requests that you come into the Medical Practice with your ID documents for your identity to be verified.

So, having read the above, please choose one of the following Options to register for Online Services:

Option 1 – Verification of ID by uploading and attaching ID Documents online

  • Step 1 – complete the Register for Online Services Form.
  • Step 2 – after completion upload and attach your ID documents file(s)
  • Step 3 – click on “Submit” which will send the form through the internet to the Practice together with the uploaded file(s)
  • Step 4 – the Registrations person will register you for Online Services.
  • Step 5 – the Registrations person will confirm your registration by email and a Letter containing your Linkage Key and Account ID.

Option 2 – Verification of ID by visiting the Medical Practice

  • Step 1 – complete the Register for Online Services Form.
  • Step 2 – click on “Submit” to submit the form
  • Step 3 – visit the Practice and present your ID documents for verification of identity:
    your online registration form will be held on the Practice’s computer system and you will not be registered until you have completed Step 3
  • Step 4 – your ID verification will be matched with your online registration form already received and held by the Practice
  • Step 5 – The Registrations person will register you for Online Services
  • Step 6 – The Registrations person will confirm your registration by email and a letter containing your Linkage Key and Account ID.

Please Note

Please do not print the Form and bring it into the Practice – this will not be accepted by Reception. The form will only be accepted by submitting online using the “Submit” button provided.

Online Registration Document Submission

To complete the registration process, we request that you attach identification. Examples of such documents may be a copy of your passport, driving license, national identity card etc. Please also submit a document confirming your postal address such as a utility bill, HMRC self-assessment letters, council tax bill, bank statement etc.
