The sharing of information aims to improve the care you receive upon unplanned admission to hospital, by assisting the clinicians who will be delivering your care with quick and secure access to your medical records held at Horsley Medical Practice.
Only a limited number of RSCH clinical staff working in A&E, Emergency Assessment Unit and Care of the Elderly Wards will be able to access your medical records to help with your care and treatment
RSCH staff treating you will ask you to provide consent to access your records when you attend the hospital. If you are unconscious or otherwise incapacitated, a clinician may decide to access your records if they believe it is in your best interests to do so to provide your care and treatment.
This system starts on 15th December 2014.
If you wish to opt out of making your information available you can do so by filling out a form at reception.
If you have opted out previously for Summary Care Records and/ or other data sharing you do not need to opt out again. Your records will not be available to RSCH under these new arrangements.
